Housing Applications can be obtained online through the links listed below and are also available at the Housing Authority’s Office.
Universal Standard Application
Universal Standard Application for State-Aided Public Housing. When you submit your application, along with the application, you must submit copies of Birth Certificates and Social Security Cards for all Household Members. To be eligible for elderly/handicapped housing you must be at least 62 years old or a person with a handicap. If you have a handicap, the handicap must be other than a history of alcohol/drug abuse. If you have a handicap, you must provide certification by a doctor clearly stating that you have a handicap and it is expected to be of long and indefinite in duration lasting at least six months. In addition, the LHA will need to determine that certain special architectural features OR low rent housing is not available in the private market AND that the applicant is faced with living in an institution or decadent substandard housing OR the applicant is paying excessive rents.

Emergency Application Instructions: (Be sure to read instructions before filling out Emergency Application) You will also need to provide other documents that the LHA needs to determine your eligibility for Emergency Case Status as well as for the program(s) for which you have applied. Your Emergency Application will not be processed until you have provided everything required.
Physician Verification Form
Physician Verification Form – Note: An applicant’s eligibility for Elderly/Handicapped Housing is contingent on the Authority being able to identify and understand whether you, the applicant, has a qualifying impairment and how it affects your housing needs. Please be sure that your Physician completes this form legibly and in a manner that allows the Authority to meaningfully evaluate your eligibility.
Checklist Emergency Application Documentation:
Please be advised that a request for priority consideration (emergency application) cannot be processed and will not be effective until such time as you have fully verified your housing circumstances and the events leading to your present situation. Until such time, the Housing Authority will process your standard application for housing. If you are found eligible pursuant to that application you will be assigned selection category 7, Standard. In doing so, should you fail to document priority status you will be on the waiting list as a Standard Applicant at the original date the LHA received your application.
